Medieval dice are cubes made from wood, minerals, bone or ivory, with each of the numbers 1 to 6 on a face. They were used in gambling and dice games, and existed well before the development of playing cards.
Loaded dice are dice weighted in some way, to cause favourable (or unfavourable) results to appear more or less often than random. Frequently the carrying of loaded dice was taken as proof of dishonesty and justification for complaint.
